Thank you for completing the permission form and health information form prior to the Oysterhaven Trip on Tuesday.
Oysterhaven Activity Centre is now asking you to fill out one final form before Tuesday. This form is in relation to COVID-19. You must complete this form prior to the trip. One form must be completed per student attending.
